Self-Donation
Self-Donation and Cremation Consent Form can be signed prior to passing if the individual is of sound mind and can make proper future decisions.
Authorizing Person Donation
Authorizing Person Donation and Cremation Authorization Form will be required to be signed after the donor is deceased, if a Self-Donation and Cremation Authorization Form has not been completed.
Donor Information
UTN recommends the following information to be completed in advance to provide further peace of mind for you and your loved ones at end-of-life. The information provided will be utilized to assist UTN in completing required end-of-life documents, including the Death Certificate Worksheet.
